   Identification: Identify the Proprietor and all signers on the account 
   Taxpayer ID Number: Owner's Social Security Number or Employer ID Number 
   Signature Card Ownership Box: Sole Proprietor 
   In-house Resolution: Proprietor or Trade Name Resolution signed by proprietor  
   Legal Documentation: Certificate of Assumed Business Name 
Non-Profit Association 
   Eligibility Description: Informal social club, charitable organization, or other non-profit association 
   Account Title Example: Association's name: Class Reunion of '72 
   Identification: Identify authorized officers and all signers 
   Taxpayer ID Number: Association's EIN 
   Signature Card Ownership Box: Non-Profit Association 
   In-house Resolution: Association Resolution signed by two officers or representatives 
   Legal Documentation: By-laws, Charter, or Minutes - if possible 
General Partnership 
   Eligibility Description: Unincorporated business owned by two or more individuals 
   Account Title Example: Partnership's name: Built Better Construction 
   Identification: Identify the general partner(s) and all signers on the account 
   Taxpayer ID Number: Partnership's EIN 
   Signature Card Ownership Box: Partnership 
   In-house Resolution: Partnership Resolution signed by all partners

Limited Partnership 
   Eligibility Description: Unincorporated business owned by two or more individuals 
   Account Title Example: Partnership's name: ABC, Ltd.

   Signature Card Ownership Box: Partnership (Limited)

Corporation 
   Eligibility Description: An incorporated business registered with the Secretary of State
